The basics your Santa needs before anything else.
- Your name. As it appears in the draw.
- Favourite colour. Useful for anything wearable or for the desk.
- Hobbies and interests. Two or three is plenty.
- Favourite way to relax. A bath, a game, a walk, a nap.
Tastes
Small preferences that turn a generic gift into a good one.
- Coffee, tea, or neither. Say how you take it if you have a strong view.
- Sweet or savoury. Name a favourite if you have one.
- What you are into right now. Books, shows, music, games, podcasts.
- Favourite shop or brand. Good fallback for a gift card.
Sizes and practicalities
Stops the two most common failures: wrong size and wrong ingredient.

- Allergies or things to avoid. Food, scent, alcohol, anything else.
- Please do not buy me. Things you already own or never use.
Wishes

Secret Santa questionnaire for work
At work, drop the sizes question and keep the rest. Colleagues rarely want to share clothing sizes with the person two desks over, and a $20 to $25 office budget rarely stretches to clothes anyway. Keep allergies: office gifts lean on chocolate, wine and scented things, and one of those is a problem for someone in every team.
Add two work-specific questions: "What would make your desk or commute better?" and "Which local coffee shop or lunch spot do you go to?" Both produce safe, specific gifts: a good desk plant, a better lunch box, a card for the place they already queue at.

Secret Santa questionnaire for family
Family draws usually have a higher budget and people who think they already know each other, which is exactly when gifts go wrong. Keep sizes in, because family is who buys you clothes. Make "Please do not buy me" mandatory: it is the question that stops a third slow cooker.
For kids, let a parent fill it in and add "Current obsession" and "Already has". For grandparents, add "Something you keep meaning to replace". For teenagers, the wishlist link question does most of the work: they will happily paste links and will not happily answer fifteen questions.
